Medical Insurance
Healthcare coverage is provided through Health Plus, Inc. All full-time employees are eligible to receive health care coverage for themselves and eligible dependents effective the first month following hire. All inpatient and outpatient care will be paid at 80-90% for services at in-network facilities after a deductible of $250 has been met. Preventative Care is provided at a maximum of $1,000 per year. Visits to your primary care physician are covered with a $15 co-pay per visit. Specialist's office services are covered with a $25 co-pay per visit. Mental health and substance abuse services are covered under the plan.
Prescription Plan
After a $100 deductible, prescription drugs cost $10 for generics, $25 for preferred brand names and $40 for non-preferred brand names. Health Plus also offers a mail order prescription plan with a cost of $20 for generics, $50 for preferred brand names, and $80 for non-preferred brand names for a 90 day supply.

Decreases taxable income by having health insurance premiums and other optional benefit costs deducted from your paycheck on a pre-tax basis.
Life Insurance
Group life insurance equal to annual salary is provided free of charge
to all full and part-time employees. Supplemental life insurance for employees,
spouses and dependent children is also available at a discounted rate.
401(K)
Employees become participants the first of the month after hire. After one year of employment, full and part-time employees are eligible for a 3% base employer contribution.
Paid Time Off
Accrual begins on employment date and is available after 30 days of employment.
Full-time employees accrue up to 136 hours the first year and 152 hours
after that. Part-time employees accrue up to 136 hours.
Disability Income Protection
Short-term disability is provided through sick time accrual. Sick time
is accrued at the rate of 5.333 hours per month for full-time employees
and 2.666 hours per month for part-time employees. A maximum
of 64 hours per year may be accrued with a maximum of 832 hours in
the sick time bank at any time. Sick time may be used after 90 days of
employment.
Long-term disability payments of 60% of basic earnings, subject to a $5,000
maximum per month, begin after 90 calendar days of continuous disability.
Differentials
All hourly employees receive a weekend differential of 15% of their base
pay. Shift differential is $1.50 for second shift and $2.00 for third shift.
