To provide quality care and services, Proctor Hospital requests that you and your family…

A grievance is a formal, written or verbal complaint that is filed by a patient, when a patient’s issues cannot be resolved promptly by staff present, or there is a violation of a patient’s rights.

To file a grievance, call Angie Moore, Patient Advocate, at 309-691-1084, or if you prefer, write your grievance and send to Angie Moore, c/o Proctor Hospital, 5409 North Knoxville Avenue, Peoria, IL 61614. The Medicare Consumer Hotline phone number is 1-800-647-8089 and may be used to report unresolved issues.
